Embaring condom question :S?

ok so me n my bf were having and he pulled out and we noticed that his *** was coming out of the top of the condom :S luckily ive been on the pill for ages so i doubt im pregnant, and he has no STDs so its all good, but i want it to never happen again. i dont wanna injure his "manhood" by telling him to get a smaller size tht fits tighter, how do i tell him to do it gently without offending him, cus he's not small lol but i just wanna be safe... and any other advice to do with condoms wud be appreciated? plz dont make fun of it -.-
