Keep hearing get a tune up what does this involve?

keep hearing americans give answers like get your engine tuned up please explain what this involves on a modern car with eobd or obd2 is it some kind of ecu software up date. i know on older cars with carbs it was adjusting the idle screw, adjusting points advancing or retarding the timing etc please explain how this can be done on a modern car
